Overview
The Medical Programs Case Coordinator provides essential support to Fora Health’s medical team, with responsibilities spanning Medication for Addiction Treatment (MAT), Residential, Withdrawal Management, and Psychiatric services. This role coordinates patient care, facilitates transitions between levels of service, and supports clinical operations. The Medical Programs Case Coordinator works closely with medical providers, nurses, residential assistants, addiction counselors, peer mentors, and administrative staff to ensure continuity and quality of care. Union Responsibilities
Care Coordination: Coordinate patient appointments, facilitate transitions between programs, occasionally accompany patients to visits, and provide personalized support including referrals, follow-ups, and resource navigation Medication Support: Assist with ordering, audits, and coordination with pharmacy partners; resolve medication-related issues and promote safe administration practices; connect patients with community resources and low-cost pharmacy options Patient & Team Communication: Liaise between medical, residential, and administrative teams; communicate with patients and providers to ensure continuity of care Documentation & Compliance: Maintain accurate records of coordination activities; ensure confidentiality, ethics, and professional boundaries Community Engagement: Provide outreach and promote access to services; collaborate with partners to support transitions and resource access Cultural Responsiveness: Apply trauma-informed, patient-centered approaches; address bicultural values and needs to improve accessibility and outcomes Professional Development: Attend required trainings and meetings; perform other duties as assigned to support program goals Additional Responsibilities
Attend staff meetings as scheduled Complete additional trainings as assigned Participate in special projects and other duties Engage in ongoing supervision and consultation If eligible, drive a company vehicle responsibly and maintain a clean driving record Work effectively as part of an inter-professional team Supervisory Functions
This position does not have supervisory duties. Qualifications
Education and Experience
Bachelor’s degree preferred Minimum 3 years of relevant experience in healthcare or behavioral health, ideally with substance use disorder and underserved populations Knowledge, Skills and Abilities
Proficient in Microsoft Office Suite (Outlook, Excel, Word) Detail-oriented and organized Effective communication across diverse audiences Ability to maintain professional boundaries Team-oriented and respectful service delivery Strong documentation skills Effective written and verbal communication Strong organizational skills; initiative and problem-solving abilities Basic knowledge of substance-use disorders Self-awareness and understanding of mental health symptoms Other Requirements
Certification of a minimum sobriety period may be required for some employees Pre-employment or for-cause drug tests Ability to pass DHS criminal background check Regular responsiveness via cell phone to work-related inquiries Working Conditions
Prolonged computer use Indoor office environment with occasional travel between sites or to events Occasional overtime or schedule adjustments may be required May involve exposure to communicable diseases and potentially emotionally challenging scenarios Exposure to cleaning chemicals with protective equipment provided Fast-paced, high-volume work environment Benefits
