Responsibilities

• Conduct psychosocial assessments of patients and their families to identify needs and develop care plans.
• Provide counseling and support to patients and their families to help cope with the emotional and social impact of illness.
• Collaborate with the interdisciplinary team to ensure comprehensive care addressing physical, emotional, and social needs.
• Advocate for patients and families to ensure their rights are respected and needs are met.
• Maintain accurate and timely documentation of all social work interventions and patient interactions.

Qualifications

• Minimum Qualifications
• Master\'s degree in Social Work from an accredited institution.
• Current LCSW license in the state of California.
• Minimum of 2 years of experience in a hospital or healthcare setting.
• Strong knowledge of psychosocial assessment and intervention techniques.
• Excellent communication and interpersonal skills.

• Preferred Qualifications
• Experience working with diverse patient populations.
• Bilingual in English and Spanish.
• Experience with electronic medical records (EMR) systems.
