Job Summary
The Hospital Social Worker (MSW) provides psychosocial support to patients and families in a hospital setting, assessing needs, developing care plans, and collaborating with healthcare teams to ensure quality care and smooth transitions.
Essential Duties and Responsibilities
- Conduct psychosocial assessments and develop care plans.
- Provide emotional support, counseling, and crisis intervention.
- Coordinate with healthcare providers and community resources.
- Facilitate discharge planning and educate patients and families.
- Assist with post-discharge services and advocate for patient needs.
- Maintain accurate documentation in EPIC.
- Address social determinants of health and connect patients with resources.
- Participate in multidisciplinary rounds and collaborate on discharge plans.
- Assist with guardianship and complex cases.
Education, Experience, and Qualifications
- MSW from an accredited institution required.
- Current licensure as a Clinical Social Worker (LCSW) preferred.
- Experience in hospital or healthcare social work preferred.
- Experience in crisis intervention and care coordination desirable.
