Role Description

This is a full-time hybrid role for a Social Worker located in Cincinnati, OH. The Social Worker will provide case management, counseling, and support to individuals and families, addressing their mental health, substance abuse, and social service needs. Responsibilities include conducting assessments, developing treatment plans, coordinating with other service providers, advocating for clients, and maintaining detailed documentation. Some work from home is acceptable, but the position requires regular on-site presence.

Qualifications

• Case Management, Counseling, and Support Services skills
• Experience in conducting Assessments and Developing Treatment Plans
• Advocacy and Coordination with Service Providers experience
• Excellent written and verbal communication skills
• Ability to work independently in a hybrid environment
• Master\'s degree in Social Work or a related field
• Licensure as a Social Worker (LSW) in the state of Ohio preferred
• Experience in mental health and substance abuse treatment is a plus
