Summary
The Behavioral Health Social Worker MSW plays a pivotal role in carrying out the mission of the organization by providing high-quality care, including treatment planning, discharge planning, and utilization review to patients hospitalized on the mental health unit. This role is responsible for assessing psychosocial needs, educating, counseling, and planning, coordinating, and implementing care across the continuum while contributing to patient outcomes and length of stay. The Behavioral Health MSW works closely with other professionals (e.g., nursing, counseling staff, occupational therapy, psychiatry, psychology). They communicate teamwork, caring, and compassion with all internal and external healthcare team members.
Responsibilities
- (25%)* Complete and document psychosocial assessments to eliminate barriers to treatment, increase patient/family satisfaction, improve resource utilization, and address issues such as safety, ethics, legal concerns, trauma, financial problems, treatment compliance, substance use, and other psychosocial barriers.
- (25%)* Provide social work interventions related to hospitalization, including crisis management, mental health and suicide resources, healthcare decision-making, ethical/legal concerns, discharge planning, and transitional care needs.
- (25%)* Participate in care coordination efforts to identify high-risk factors, respond appropriately, and co-lead daily care coordination meetings with the care team.
- (25%)* Collaborate with the healthcare team, patient, and family to transition the patient to appropriate levels of care, documenting and advocating for the patient, and formulating plans to maintain health and prevent readmission.
Additional Duties
- Integrate theories of family dynamics, crisis intervention, strengths-based, and solution-focused approaches into patient work.
- Participate in care coordination, collaborate with outside agencies, and build professional relationships with patients and families.
- Provide excellent customer service, maintain licensure, and meet age-specific needs according to standards and policies.
- Other duties as assigned.
Qualifications
- MSW from an accredited program and licensure as a Master of Social Work in Michigan or actively working towards it.
- Certifications: BLS, TEAM ADVANCED, Fingerprinting, iChat, Recipient Rights, and others as specified.
- Experience and skills in crisis intervention, assessment, community resources, and cultural competence.
- Must adhere to ethical standards and organizational policies.
Physical and Mental Requirements
Ability to handle stressful situations, move freely within the facility, and perform physical tasks as outlined. Must meet physical demands and safety requirements.
