Summary
The Behavioral Health Social Worker MSW plays a pivotal role in providing high-quality care, including treatment planning, discharge planning, and utilization review for patients hospitalized on the mental health unit. This role involves assessing psycho-social needs, educating, counseling, and coordinating care across the continuum to improve patient outcomes and length of stay. The MSW works closely with healthcare professionals such as nursing, counseling staff, occupational therapy, psychiatry, and psychology, demonstrating teamwork, caring, and compassion.
Responsibilities
- Complete and document psycho-social assessments to eliminate treatment barriers, increase patient/family satisfaction, and optimize resource utilization.
- Provide intervention related to hospitalization, crisis management, mental health, and discharge planning.
- Participate in care coordination to identify high-risk factors and respond appropriately, co-leading daily team meetings.
- Work collaboratively with the healthcare team, patients, and families to transition patients to appropriate care levels, advocate, mediate, and develop comprehensive care plans.
Additional duties include integrating relevant theories, participating in team meetings, collaborating with outside agencies, building professional relationships, providing excellent customer service, and maintaining licensure and professional development.
Qualifications
- MSW (Masters of Social Work) and licensure as a Master's Social Worker in Michigan or actively working towards it.
- Certifications such as ICHAT, TEAM ADVANCED, and Fingerprinting as specified.
- Experience in casework principles, crisis intervention, motivational interviewing, and community resources.
- Knowledge of social work practices in behavioral health and adherence to NASW Code of Ethics.
Physical/Mental Requirements
Exposure to stressful situations, ability to wear PPE, move freely, and perform physical tasks as outlined. The role is classified as Light physical demand.
Additional Information
