Holy Cross Health is seeking a strategic and experienced
Director of Documentation Quality and Care Transitions
to lead a system-wide program focused on improving documentation, reducing readmissions, and optimizing resource utilization. This role is responsible for developing and directing initiatives that support denials prevention, clinical documentation improvement, palliative care, and care transitions. The Director will collaborate across departments and facilities to ensure high-quality, cost-effective care delivery aligned with the mission of Trinity Health and Holy Cross Health. What You Will Do
Lead and manage integrated programs for documentation quality, readmission prevention, and care transitions across Holy Cross Hospital, Holy Cross Germantown Hospital, and Holy Cross Network. Supervise staff in areas including palliative care, clinical documentation improvement, denial prevention, and readmission management. Oversee compliance with regulations affecting patient status and insurance, and guide staff interactions with external agencies. Collaborate with the Medical Director of Care Management and other leaders to implement departmental policies and initiatives. Provide education and consultation to clinical staff, medical staff, patients, and families to support documentation and care transition goals. Analyze data at both individual and aggregate levels to identify trends and drive performance improvement. Promote quality outcomes, patient satisfaction, and cost-effective care through coordinated efforts and strategic planning. Minimum Qualifications
Education:
Master’s degree or higher in a healthcare-related field (e.g., Nursing, Social Work, Healthcare Administration, Medicine) required. Licensure:
Must maintain licensure recognized by the State of Maryland if trained in a clinical specialty. Experience:
Minimum of 5 years in care management or a related field; at least 2 years of leadership experience in case management, utilization review, or a closely related area; experience with clinical documentation improvement initiatives strongly preferred. Skills:
Extensive knowledge of payer mechanisms and clinical utilization management; strong interpersonal and team collaboration skills; proven ability to lead through influence and build effective relationships across all levels; familiarity with regulatory and accreditation standards related to discharge planning and resource utilization; proficiency in computer systems and data analysis tools.
