Location: Parma Heights, Ohio (Broadview Nursing Home - 5220 Broadview Road, Parma, OH 44134) and related SNF dialysis locations.

Responsibilities

• Support DaVita Skilled Nursing Facility Dialysis patients. The Social Worker must be able to visit each site.
• Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.
• Be part of a team that appreciates, supports and relies on each other in a positive environment.
• Guide patients in understanding their rights and responsibilities, and help manage the physical, mental, emotional and financial demands of End Stage Renal Disease (ESRD).
• Collaborate with sites supported by the SNF dialysis program, including Parkside Villa, Andover Village Retirement Community, St. Augustine Health Campus, Broadview Nursing Home, and Premier Washington Rehabilitation and Nursing Center.

Requirements

• Meets all state regulations to practice in the dialysis setting.
• Master's degree in Social Work (MSW) with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook; proficiency with DaVita-specific applications within 60 days.
