Overall Summary:
The Community Health Worker (CHW) I at Bay Area Community Health will primarily work within the harm reduction program, serving and assisting our patients while located at different sites throughout the organization and community events. The Community Health Worker (CHW) will work at different capacities based on their assigned area of training and specialty. The CHW will be a trusted member of the community, and actively participate as a liaison between primary care, specialties and resources available to BACH patients. Activities may include, but are not limited to, outreach, engagement, care coordination, case management, patient education, health coaching, linkage of the patient with resources at BACH and in the community such as food, housing, financial and transportation assistance and also linking the patient to care within the clinic.

Under the general direction of the supervisor/manager, the Community Health Worker performs a variety of services. Duties include: Deliver and implement harm reduction services and overdose prevention services to underserved populations. Deliver and implement harm reduction education (e.g., safer drug use, vein care) Conduct street and community based outreach to distribute harm reduction supplies (naloxone, safer use kits, condoms) Build trusting relationships with people who use drugs and other marginalized groups through regular contact and follow up Offer education on safer drug use, overdose prevention, sexual health and health related topics. Provide referrals and navigation support for medical , mental health, housing, and substance use services Assist individuals with overdose response training and naloxone distribution Work closely with community partners, shelters, clinics, and advocacy groups Support data collection, documentation, and reporting as required by funding and program evaluation needs Participate in harm reduction trainings, team meetings, and supervision Link patients to services within BACH and the broader community. Participate in staff team meetings, case conferences and training. Collect data and provides documentation in client files as required by agency and funders. Participate in obtaining deliverables required for various grants, such as, but not limited to, eligibility assessment, socioeconomic needs assessment, and medical disparities. Assist families with immediate crisis intervention, advocacy, problem-solving and other appropriate interventions, as needed Utilize de-escalation techniques in situations as appropriate Utilize program evaluation tools (i.e., pre/ post-tests, satisfaction surveys, etc.), analyzing and reporting on outcomes. Provide resources for various health topics, such as nutrition, physical activity, hygiene, sexual health, substance use, and harm reduction strategies Work flexible hours, which includes some evenings and weekends Work at any of BACH location, including clinics, mobiles, schools, homeless encampments, warming centers and patients' homes, or patient appointments as assigned Completion of CHW Certification after 2,000 hours of work Book appointments in Electronic Health Record, according to providers' scope of practice. Use Microsoft Office to communicate and report information Perform patient screening surveys; score questionnaires and alert care team Document encounters in electronic health records Participate in health fairs, community and outreach events to promote BACH services Maintain logs as directed by supervisors Participate in health fairs, tabling, collect information Follow-up on no-show appointments Schedule in person interpreters, and transportation, via patient health plans as applicable Be highly dependable and independent- Conduct outreach and enrollment services in available public benefit programs Connect patients to internal resources and/or services to meet their needs Serve as a liaison between the community and health and social services. Interface well with clinic staff, community and care teams. Meet departmental review and documentation standards for work assignment Adhere to the policy and procedures of assigned clinic(s) Build and maintain appropriate relationships on behalf of BACH Support a positive work environment and foster teamwork Support a positive image of BACH inside and outside the organization. Work flexible hours, which includes some evenings and weekends Work at any of BACH location, including clinics, mobiles, schools, homeless encampments, warming centers and patients' homes, or patient appointments as assigned Secondary Responsibilities: Attends workshops, training, and meetings as needed, and as requested. Performs other duties as assigned by supervisor. Qualifications: Required Education, Experience, and Training High School Diploma or equivalent Two (2) years of experience in a community-based health care service capacity preferred Bilingual preferred Work Conditions Travel to other sites and/or locations required Work outside in varying temperatures may be required Prolonged periods of standing or sitting. Work flexible hours, which includes some evenings and weekends PhysicalDemands: This position requires: Sitting, walking, and standing in a typical medical/clinical office environment. Extended and/or prolonged periods of sitting, walking, or standing. Regular bending, stretching, and lifting up to 50 pounds. Regularly reaching with arms, climbing, balancing, stooping, kneeling, crouching, and crawling while performing job duties. Moderate exposure to unusual elements, such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and/or loud noises. Manual dexterity for tasks involving standard office equipment and computer keyboard, including repetitive movements such as typing. Traveling to work sites, including exposure to outside elements. Exposure to potentially dangerous materials, hazards, physical risks and situations, requiring adherence to extensive safety precautions and possibly the use of protective equipment.
