Nursing Supervisor

The purpose of the Nursing Supervisor is to direct nursing staff activities for the wellbeing of Pleasant View Nursing Home residents. This position is for the PM shift. The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned. Provide leadership to the nursing team in a professional and positive manner. Work closely with the nurses, nursing assistants, medication assistants and unit clerks to establish a positive working environment. Demonstrate conduct that is reflective of team collaboration and support. Communicate concerns and/or problems with the DON in a timely manner proposing possible solutions or remedies to the concerns and/or problems. Be supportive of the final solution. Direct nursing activities on the Night shift. Assign and monitor nursing staff tasks in accordance with current regulations, best practice nursing standards and Pleasant View guidelines. Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of the residents' care. Perform duties such as completion of the assessment and care planning process, and follow through of physician rounds. Provide requested reports for the facilitys quality assurance program and submits them to the DON in a timely manner. Participate in surveys (inspections) made by authorized government agencies. Make written and oral reports recommendations to the DON as necessary required, concerning the operation of the Nursing Department. Follow through with the admission, transfer, and discharge process in cooperation with the NCC/Clinical Assessment Nurse and the Social Worker. Complete accident/incident and skin injury reports as necessary; reviewing them for completeness. Complete the investigation of the root cause. Monitor the follow-up review of the care plan for effective and appropriate interventions. Attend various committee meetings of the facility, as required. Communicate with Medical Providers when there are changes in condition and decline in condition. Report and investigate all allegations of resident abuse, neglect, misappropriation of property, resident to resident altercation or injuries of unknown origin. As assigned will submit the online report to BQA. As assigned will complete the final report for submission to BQA. Share weekend on-call rotation with other nursing management staff. Complete other duties as assigned by the DON. Personnel Functions Assist the DON in the development of nursing department goals and plans. Actively support the goals and steps to achieve the goals in a collaborative manner. Make daily rounds observing that nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Present findings in a timely manner to the employee providing necessary education and discipline as necessary. Report these findings and the action taken to the DON. Assist as needed with the replacement of a call-in. Review complaints and grievances made or filed by department personnel. Make appropriate reports to the DON as required or as may be necessary. Present disciplinary action in a private setting and administer the action in a professional manner. Establish a good understanding with the employee in regard to expectations. Develop and maintain a good working rapport with personnel within the nursing department and other departments within the facility. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. Meet with the team on a regularly scheduled basis to promote effective communication, timely problem solving, and the improvement of services. Attend all mandatory meetings and in-services and complete the make-up session in a timely manner. Encourage night shift attendance as well. Monitor that privacy and confidentiality of resident information is upheld. Monitor that facility computers are used for resident related services only. Monitor that employee cell phones are kept in the employee locker room or employee vehicle. Nursing Care Functions Inform nursing service personnel of new admissions and information about the new resident. Audit the appearance of the unit, including kitchenette, med room, shower room, all supply and utility rooms. Review nurses documentation monitoring that it is descriptive of the nursing care being provided and reflective of the residents response to care. Complete resident assessments as needed to observe and evaluate the residents physical and emotional status. Monitor medication administration and treatment schedules observing that they are being administered as ordered and provided as scheduled. Review medication record for completeness of information, accuracy in the transcription of physician orders, and adherence to stop order guidelines. Update the DON and the Administrator of the status of residents as necessary. Inspect the nursing service treatment areas to monitor that they are maintained in a clean and safe manner. Notify the residents attending physician and HCPOA when there is a significant change in the residents condition. Start IVs, obtain sputum, urine and other lab tests as ordered. Admit, transfer and discharge residents as necessary. Assist the Unit Nurse in monitoring seriously ill residents. Inform family members of the death of resident as necessary. Safety and Sanitation Monitor your shifts personnel observing that they are following established safety regulations in the use of equipment and supplies. Monitor that nursing service work areas are maintained in a clean and sanitary manner. Monitor that all resident rooms, treatment areas, etc., are maintained in a clean, safe, and sanitary manner. Monitor that nursing service personnel participate in fire safety and disaster preparedness drills in a safe and professional manner. Monitor that nursing service personnel follow infection control guidelines. Monitor that nursing service personnel follow established hand washing guidelines. Care Plan and Assessment Functions Participate in the development of written preliminary and comprehensive assessments of the nursing needs of each resident as necessary. Participate in the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care. Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs. Observe that personnel involved in providing care to the resident is aware of the care plan and that care plans are used in administering daily care to the resident. Complete regular audits of the process for managing resident change of condition, observing that the process is complete and completing aspects of the process that have not been completed. Prepare the respective written report on the audit tool. Resident Rights Monitor that staff are quiet as they make rounds and provide care. Monitor that residents who request that they not be disturbed during the night are respected. Maintain the confidentiality of all resident personal health information. Monitor that residents are treated fairly, and with kindness, dignity, and respect. Monitor that nursing care is provided privately and that nursing service personnel knock before entering the residents room. Monitor that nursing service personnel are knowledgeable of the residents rights. Minimum Training and Experience Required to Perform Essential Job Functions Education and/or Experience Bachelor or Associates degree or diploma in nursing. Valid Wisconsin RN license Five years nursing experience and two years in a supervisory capacity, or any combination of education and experience that provides equivalent knowledge, skills, and abilities. Geriatric nursing experience preferred. An excellent attendance record is required.
