Position Summary

We have an exciting opportunity to join our team as a Registered Nurse Full Time - night.

In this role, the successful candidate provides professional nursing care in accordance with physician treatment plans to support the comfort and well-being of patients and their families. The nurse assesses needs, plans and implements the treatment plan, observes and evaluates progress, and documents pertinent observations. Works collaboratively with the interdisciplinary health team, directs the delivery of nursing care by other nursing staff, and participates in orientation of new employees.

Responsibilities

  • The registered nurse collects patient health data by involving the patient, family and other health care providers.
  • Performs initial assessment based on the patient’s presentation.
  • Records relevant data on the nursing component of the Interdisciplinary Admission Assessment.
  • Demonstrates awareness of the special needs of each age group or culture served as well as patients with disabilities.
  • Performs ongoing assessments as established by department policy and as warranted by the patient's response.
  • Incorporates discharge planning into the admission process.
  • Analyzes assessment data to identify problems and determine patient-centered outcomes.
  • Demonstrates the ability to combine theoretical knowledge and clinical expertise to anticipate and plan for identified care needs.
  • Identifies actual or potential problems and validates them with the patient, family and other health care providers.
  • Identifies cultural, ethnic, and spiritual aspects of patient/family dynamics related to care.
  • Recognizes patient comfort/pain management needs impacting the plan of care and outcomes.
  • Identifies knowledge deficits for the patient and/or family and communicates plan of care to promote continuity of care.
  • Develops and communicates a plan of care and collaborates with patient, family and appropriate healthcare providers.
  • Implements a family-centered plan of care aligned with physician treatment plans and integrates cultural, ethnic and spiritual aspects of care.
  • Incorporates teaching and learning principles based on identified learning needs and developmental level.
  • Assists patient and family in identifying and securing services to address health-related needs.
  • Documents initial plan of care and provides updates as required; sets realistic goals and priorities; uses time efficiently.
  • Monitors and adapts the plan of care for each patient; provides education to patient and family.
  • Maintains patient safety and creates a safe environment; collaborates with team to implement interventions.
  • Assigns or delegates tasks per state nurse practice act and caregiver competencies.
  • Demonstrates knowledge of medication administration, including contraindications and adverse effects; seeks resources when uncertain.
  • Adheres to medication policy for order transcription/verification, administration and documentation.
  • Evaluates progress toward outcomes and documents patient responses and revisions to the plan of care.
  • Uses ongoing assessment data to revise outcomes and care plans as needed; identifies barriers and mitigates impact on patient/family.
  • Communicates with and includes the patient, family and other health care providers in the evaluation process.
  • Delivers care with ethical considerations and respect for patient rights; supports customer satisfaction and quality improvement in nursing practices.
  • Maintains effective interactions with unit staff and participates in professional development of peers and staff.
  • Act as a facilitator when addressing issues concerning patient care and unit functioning; seeks assistance from Nurse Manager or Nursing Administrative Supervisor when needed.
  • Accepts feedback for improved patient care and unit functioning; engages in performance appraisal to identify strengths and development areas.
  • Collaborates with management to improve the environment of care for patients, families and personnel.
  • Maintains knowledge of principles, policies and procedures specific to the assigned nursing service; participates in ongoing learning activities.
  • Complies with staff meeting attendance requirements.

Minimum Qualifications

  • Professional knowledge of nursing theory and practice acquired through completion of a two-year program at an accredited School of Nursing.
  • Current New York State license or permit as a Registered Professional Nurse.
  • Interpersonal skills to interact effectively with patients and families; demonstrates cultural competence.
  • Commitment to customer service excellence and clear verbal and written communication.
  • Analytical ability to assess and develop solutions to clinical situations or unit problems.
  • Maintains knowledge and proficiency in the clinical area and age group served.
  • BLS certification and de-escalation training; ACLS (ICU, Float pool).
  • Ability to frequently lift/carry more than 75 lbs; push/pull more than 100 lbs; bend, squat, kneel, climb stairs/ladder and reach above shoulders.

Required Licenses: Registered Nurse License-NYS

Long Island Community Hospital provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $97,440.00 - $121,680.00 annually. Actual salaries depend on experience, specialty, education, and hospital need. The salary range listed does not include bonuses or other forms of compensation.
