Overview

Base pay range: $67,000.00/yr - $86,000.00/yr. The Social Worker is an experienced professional and member of the patient care team who provides emotional and practical support for patients and their families in accordance with their Plan of Care. As part of the patient care team, the Social Worker meets with patients and their families, coordinates with the clinical team, and connects them with community resources and services.

Responsibilities

  • Assess the psychosocial status of patients and families/caregivers related to the patient\'s terminal illness and environment; communicate findings to the Registered Nurse and other members of the interdisciplinary care team. Include home assessments as needed and arrange alternative plans if the home environment is not safe.
  • Conduct social evaluations, including family dynamics, caregiver abilities, communication patterns, suicide risk, neglect, or abuse, and plan interventions based on evaluation findings.
  • Counsel patients and families/caregivers as needed in relation to stress and other coping difficulties; provide crisis intervention when necessary.
  • Provide ongoing bereavement support to individuals identified as moderate or high-risk.
  • Assess cultural needs of patients and families; educate the interdisciplinary care team on cultural considerations, including communication, family roles, space, and traditions.
  • Work under the supervision of a Master\'s Prepared Social Worker (MSW).
  • Maintain clinical records on all patients referred to social work.
  • Educate patients and families and assist in the preparation of advanced directives.
  • Provide information and referral services for practical and environmental needs; act as liaison to community agencies.
  • Maintain and develop contracts with public and private agencies as resources for patients and personnel.
  • Participate in development of the individualized plan of care with patient and family; attend regularly scheduled interdisciplinary group meetings and help the team recognize psychosocial stress effects on symptoms.
  • Assist physicians and other team members in understanding significant social and emotional factors related to health problems and end-of-life issues.
  • Assist Bereavement leadership in maintaining a program to meet the needs of families/caregivers for 13 months following a patient\'s death, ensuring compliance with state, federal, and CHAP regulatory requirements.
  • Ensure timely completion of the initial bereavement assessment or assignment to an appropriate team member.
  • Assist Chaplain in making condolence calls to the bereaved as needed.
  • Assist with maintaining, organizing, and updating bereavement support group schedules.
  • Assist families and patients in planning funeral arrangements and related financial, legal, and health care decision responsibilities.
  • Maintain compliance with all HIPAA regulations and protect patient information on a minimum necessary basis.

Qualifications

Required

  • Bachelor’s degree in social work from an accredited Council on Social Work Education program.
  • Current license in the state(s) of practice (LSW or LCSW); two licensure levels: Licensed Social Worker (LSW) and Licensed Clinical Social Worker (LCSW). Social workers may possess compact licenses.
  • Ability to pass background, consumer report, and drug tests.
  • Valid State Driver’s License, access to a vehicle in good working order, and appropriate automobile insurance; acceptable driving record required.
  • Licensure in Iowa also for training purposes; travel to Rockford, IL for training may be needed.

Preferred

  • Master\'s degree in social work (preferred).
  • 1+ years of social work experience in a health care setting.
