Overview

The Social Worker role at Jewish Board of Family and Children's Services provides clinical intervention within the Adult Residential Division, supporting adults with serious and persistent mental illness to live in the community with as much independence as possible. Services are culturally competent, non-judgmental, and based on harm-reduction principles to help residents meet personalized recovery goals, develop independent living skills, and live in a healthy and secure environment.

The Social Worker works as part of a therapeutic team with the agency’s medical, psychiatric, and psychology staff to ensure appropriate treatment interventions for clients.

Position overview

The Social Worker is responsible for delivering necessary clinical interventions within the program, providing culturally competent and sensitive services, and collaborating with the division and agency staff to ensure appropriate treatment interventions for residents.

Key responsibilities

  • Carries a caseload of residents and provides services within program regulations and agency mission.
  • Completes client assessments, including intake, overall client assessments, and risk assessments.
  • Evaluates and supports clients in meeting individual goals; conducts person-centered and collaborative service planning; ensures service plans are completed and signed by residents per regulatory guidelines.
  • Interprets and explains agency policies and procedures to residents.
  • Develops safety plans and provides individual and family counseling as appropriate.
  • Makes referrals and coordinates services (medical, mental health, substance use prevention and treatment) and advocates on behalf of residents with other service providers.
  • Links residents to community-based programs and collaborates with other service providers; may run groups under supervision.
  • Participates in audits and other duties as assigned.

Team process

  • Participates in weekly case conferences and intake, assessment, discharge, and follow-up conferences.
  • Participates in regular staff meetings and trainings; may travel to offsite meetings and trainings.
  • Participates in planning and execution of special and holiday events.

Documentation

  • Files incident reports as appropriate; completes regular and timely documentation of all resident interactions, including progress notes.
  • Facilitates monthly roommate meetings when appropriate.

Competencies

  • Excellent engagement and assessment skills; strong verbal and written communication; attention to detail.
  • Ability to work independently and as part of a team; crisis management and de-escalation skills; good judgment and problem-solving abilities.
  • Compassion, respect for vulnerable individuals, and openness to anti-racist practices; cultural sensitivity; ability to stay calm in stressful situations.

Educational / training requirements

  • Master’s degree in Social Work; LMSW or LCSW certification preferred
  • Minimum of 1 year experience in Human Services preferred

Additional qualifications

  • Willingness to work with individuals with experiences of mental illness and substance use; compassion and respect for vulnerable individuals; patience with stabilization and recovery.
  • Familiarity with Windows environment; ability to learn new systems and software.

Physical/working conditions

  • Travel in the community as required.
