Ongoing training, career advancement programs, tuition reimbursement, and leadership development Eliot's Community Service Agency (CSA) serves children and youth (through age 21) and their families who are in need of support in managing and coordinating services. Eliot is excited to align these essential CSA services with the catchment area of our established Community Behavioral Health Centers (CBHCs) to offer a seamless and effective community-based continuum of care. The ideal candidate for the Family Partner - CSA

position is a compassionate and experienced parent or caregiver of a youth with special needs, particularly in behavioral health, who has firsthand knowledge of navigating child and family service systems. They possess strong communication, advocacy, and collaboration skills, and are committed to empowering families while supporting them in accessing community resources and achieving their care plan goals. Responsibilities: Work closely with the CSA team, family, and other collaterals in developing measurable family partner goals to support the familys Individual Care Plan (ICP) and related goals. Engage caregiver(s) in activities in the home and in the community that are designed to address one or more goals/tasks on the familys ICP. Assist caregiver(s) in meeting the needs of the youth through educating, supporting, coaching, modeling, advocating and guiding. Teach caregiver(s) how to network/link to community resources and treatment providers. Support caregiver(s) in advocating for services and resources to meet youth and family needs. Guide, support and foster empowerment including, linkages to individual, peer/parent support and self-help groups. Qualifications: Associates degree in human services or related field Must have a valid drivers license and reliable transportation Must be a parent, guardian or caregiver of a youth with behavioral health needs Experience in navigating child/family service systems Schedule: Monday- Friday, 9am-5pm
