Overview

The Boston Medical Center Geriatrics Ambulatory Practice provides primary medical care and nursing care management to adults age 65 and older as well as Comprehensive Geriatrics Assessment and Memory Disorder Assessment as consultative services. This nursing position is a unique one requiring a variety of specialized nursing skills to effectively to manage the complex medical and psychosocial needs of older adults within an interdisciplinary team.

This RN outpatient position includes in person nurse visits for assessment and management of both sub-acute and chronic medical problems; vaccine and medication administration, wound careas well as patient teaching regardingtheir medical conditions,medications and overall care plan. In addition a large portion of responsibilityis dedicated to telephone triage and care management.

Responsibilities

• Provide in-person nurse visits for assessment and management of sub-acute and chronic medical problems.
• Administer vaccines and medications; perform wound care; provide patient teaching regarding medical conditions, medications, and overall care plan.
• Lead telephone triage and care management to support acute and ongoing needs of a high-risk elder population.
• Collaborate with an interdisciplinary team to address complex medical and psychosocial needs.

Qualifications

• Broad fund of clinical knowledge and skills.
• Specific geriatrics nursing knowledge and skills.
• Keen telephone triaging skills to evaluate episodic acute and chronic clinical issues with critical reasoning and clinical judgment to expedite appropriate care.
• Broad knowledge of community resources.
• Effective communication skills with patients, families and caregivers as well as an interdisciplinary team.
• Strong organization skills with the ability to multitask.
• Expertise in problem solving and decision-making.
• BSN and 5 years of clinical experience as an RN functioning in a model of care delivery system that included care management. Experience working with Geriatric population is preferred.

Work schedule 7:30a-6p, 4 days per week

Clinic is open Monday-Friday
