Staff Nurse-Geriatrics role at Boston Medical Center (BMC)

The Boston Medical Center Geriatrics Ambulatory Practice provides primary medical care and nursing care management to adults age 65 and older as well as Comprehensive Geriatrics Assessment and Memory Disorder Assessment as consultative services. This nursing position is a unique one requiring a variety of specialized nursing skills to effectively to manage the complex medical and psychosocial needs of older adults within an interdisciplinary team.

This RN outpatient position includes in person nurse visits for assessment and management of both sub-acute and chronic medical problems; vaccine and medication administration, wound care as well as patient teaching regarding their medical conditions, medications and overall care plan. In addition a large portion of responsibility is dedicated to telephone triage and care management.

Responsibilities

• Provide in person nurse visits for assessment and management of sub-acute and chronic medical problems in older adults.
• Administer vaccines and medications; perform wound care; provide patient teaching regarding medical conditions, medications, and overall care plan.
• Conduct telephone triage and engage in care management to support high-risk geriatric patients.
• Collaborate with an interdisciplinary team to address complex medical and psychosocial needs of older adults.

Qualifications

• Broad fund of clinical knowledge and skills
• Specific geriatrics nursing knowledge and skills
• Keen telephone triaging skills with critical reasoning and clinical judgment
• Broad knowledge of community resources
• Effective communication with patients, families, caregivers, and an interdisciplinary team
• Strong organization skills with the ability to multitask
• Expertise in problem solving and decision-making
• BSN and 5 years of clinical RN experience in a model of care delivery that included care management; experience with geriatric population preferred

Schedule and Other Details

Work schedule: 7:30a-6p, 4 days per week. Clinic is open Monday–Friday.
