Responsibilities (Essential Job Functions)
- Attend work on a regular and predictable schedule; maintain data entry in the CSIL system and HEART; answer toll-free intake line for interest list calls; handle inquiries and route messages as necessary. (Approximately 75%)
- Perform interest list releases, track release responses, and follow up with released individuals to ensure timely responses. (Approximately 5%)
- Coordinate sharing forms and documents for the interest list between organizational units and with regional/state office staff.
- Prepare and type correspondence, form letters, and memorandums related to the interest list.
- Perform interest list contacts, send letters to correct addresses, and research undeliverable mail using SAS and TIERS.
- Complete additional tasks or projects as assigned by management.
Knowledge, Skills And Abilities (KSAs)
- Knowledge: Knowledge of community resources available to persons who are elderly or have disabilities.
- Skills: Creating and updating Excel spreadsheets.
- Abilities: Explain complex information clearly; read and interpret information; write instructions and develop timelines; enter information while speaking on the phone; conduct research and prepare detailed reports.
Qualifications
Initial Screening Criteria: High School Diploma or GED required. Experience with Health and Human Services waiver services or experience with children or adults with disabilities. Preferred experience handling a high volume of assignments in a short period and proficiency with a PC and Microsoft software.
