POSITION: Social Worker
DEPARTMENT: Social Services
RESPONSIBLE TO: Director of Social Services
DESCRIPTION: Social Services are available to all residents of The Virginia Home and their friends/families. The Social Worker is responsible for monitoring the resident's mental and psychosocial needs and providing the services needed for the resident to attain or maintain the highest practical level of physical, mental, and psychosocial well-being.
Job Duties
- Advocacy
- Work with the interdisciplinary team and administration to promote and protect resident rights and the psychological wellbeing of each resident.
- Prevent and address resident abuse as mandated by law and professional licensure.
- Advocate for residents in conflict situations and assist in the grievance process.
- Case Management
- Provide case management to a caseload of ~60 residents to include, but is not limited to, assisting residents with making online purchases, assist with absentee voting, assist with providing information regarding jury duty, assisting residents in completing tasks they are unable to complete independently, etc.
- Document progress in meeting the psychosocial needs of residents using the electronic medical record.
- Assist residents and caregivers in completing Medicaid redeterminations.
- Coordinate the resident discharge planning process and make referrals for appropriate services.
- Coordinate with family members and chaplain when a resident dies. Offer grief support.
- Clinical
- Complete a social history and psychosocial assessment for each newly admitted resident which identifies social, emotional, and psychological needs.
- Evaluate and assess each resident quarterly and provide appropriate documentation in the resident’s chart.
- Participate in resident care plan conferences and contribute pertinent psychosocial information.
- Lead therapeutic and education social service programs and groups for residents, families, and significant others.
- Coordinate and participate in advance care planning discussions.
- Serve as a resource for the behavioral health team.
- Serve as liaison and coordinate care between residents, mental health providers, and facility care team.
- Education
- Educate staff regarding the role of the social worker in the facility and the psychosocial needs of the residents and their families or significant others including the problems of aging, disability, and disease progression.
- Work in close collaboration with physicians and other health care personnel in resident evaluation and treatment to further their understanding of significant social and emotional factors underlying residents' health problems.

- Must be pleasant and able to relate patiently with residents, their families/significant others, volunteers, colleagues, and board members.
- Ability to manage time to complete assigned duties.
- Skilled in communication, assessment, and social work methods and techniques.
- Able to work effectively with a variety of disciplines in an individual and team setting.
- Proficient in Microsoft Office, electronic communication, and an electronic medical record.
- Communicates in English, both verbally and in writing, and possess excellent communication and interpersonal skills to perform the tasks required.
- Exhibits tact, diplomacy, integrity, intelligence, and compassion.
- Organized and able to handle multiple priorities with strong attention to detail and initiative.
- Demonstrates a confident and positive attitude and enjoys interfacing with residents and staff.
- Must consent to TST (tuberculin skin test) and Flu vaccine, pass a criminal background check, and pre-employment drug screen.
- Able to comply with facility infection prevention measures.
- Training and Experience
- Master’s degree in social work required from an accredited institution.
- Minimum 2 years’ experience serving in a long-term care setting.
Physical Demands:
- Lifting and carrying short distances up to 10 pounds occasionally.
- Able to withstand heat/cold of residents’ rooms.
- Walking short to moderate distances, including stairs.
- Reaching below knees or overhead occasionally.
- Able to perform duties with reasonable accommodation.
- Typical work schedule is Monday through Friday. However, occasional nights/weekends work required.
