Social Services are available to all residents of The Virginia Home and their friends/families. The Social Worker is responsible for monitoring the resident's mental and psychosocial needs and providing the services needed for the resident to attain or maintain the highest practical level of physical, mental, and psychosocial well-being.
Responsibilities
- Administrative: Review facility policies and procedures to assure compliance with state and federal regulations; participate in reviewing and setting policies concerning resident care and quality of life; participate in developing facility social work policies; understand and meet all government requirements for social service documentation; develop and maintain good working relationships with outside agencies; maintain confidentiality of all resident health information to ensure residents’ rights are protected.
- Advocacy: Work with the interdisciplinary team and administration to promote and protect resident rights and the psychological wellbeing of each resident; prevent and address resident abuse as mandated by law and professional licensure; advocate for residents in conflict situations and assist in the grievance process.
- Case Management: Provide case management to a caseload of ~60 residents including assisting residents with making online purchases, assist with absentee voting, assist with providing information regarding jury duty, assisting residents in completing tasks they are unable to complete independently; document progress in meeting the psychosocial needs of residents using the electronic medical record; provide information to resident/families regarding Medicaid and other financial assistance programs available; assist residents and caregivers in completing Medicaid redeterminations; coordinate the resident discharge planning process and make referrals for appropriate services; coordinate with family members and chaplain when a resident dies and offer grief support; visit residents when they are hospitalized to provide company, emotional support, and assistance in contacting family; assist with seeking guardianship.
- Clinical: Complete a social history and psychosocial assessment for each newly admitted resident; evaluate and assess each resident quarterly and provide documentation; participate in the development of a written, interdisciplinary plan of care; work with residents to identify psychosocial needs, develop plans to address them, and monitor treatment effectiveness; participate in resident care plan conferences and contribute pertinent psychosocial information; ensure or provide therapeutic interventions; assist residents in coping with transition to long-term care; provide support and education to residents and families; lead therapeutic and education social service programs and groups; coordinate advance care planning discussions; review behavioral health documentation and address behaviors; serve as a resource for the behavioral health team; liaise and coordinate care between resident, mental health provider, and facility care team; serve as Resident Care Assistant in the event of a disaster or extreme situation.
- Education: Educate staff regarding the role of the social worker and psychosocial needs of residents and families; collaborate with physicians and health care personnel; educate staff on cultural diversity, residents’ rights, and grievance policies; educate residents and families on rights and responsibilities, grievance processes, and problem-solving; supervise students assigned to social services in accordance with school guidelines and monitor their progress.
- Aptitudes and Skills: Build relationships and trust with residents, families, volunteers, colleagues, and board members; be pleasant and patient; manage time effectively; be skilled in communication, assessment, and social work methods; work with diverse disciplines in individual and team settings; understand social systems and implement state and federal regulations; proficient in Microsoft Office, electronic communication, and electronic medical records; communicate in English with excellent written and verbal skills; demonstrate tact, diplomacy, integrity, intelligence, and compassion; stay organized with multiple priorities; maintain a positive attitude and engage with residents and staff; consent to TST, pass a criminal background check, and pre-employment drug screen; comply with infection prevention measures.
- Training and Experience: Master’s degree in Social Work required from an accredited institution; minimum 2 years’ experience in a long-term care setting.
- Physical Demands: Lifting and carrying up to 10 pounds occasionally; withstand temperature variations in residents’ rooms; sitting frequently; walking short to moderate distances including stairs; reaching below knees or overhead occasionally; grasping and writing frequently; talking and hearing in person and on the phone; perform duties with reasonable accommodation; typical work schedule is Monday through Friday with occasional nights/weekends.
