Description
Primary Function:
Under the direction of the Chief Nursing Officer, the Inpatient Social Worker provides patient assessment, treatment planning, and discharge planning.
Essential Duties and Responsibilities:
- Provides psychosocial assessments of patients to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.
- Assists in identifying high-risk patients and provides case management services.
- Formulates, develops, and implements discharge plans utilizing appropriate social work interventions.
- Participates and represents social work perspectives in multidisciplinary teams, rounds, etc.
- Assesses the psychosocial needs of patients needing subsequent vision care/treatment.
- Evaluates, tracks, and attempts to eliminate or lessen barriers and/or attitudes to care that often preclude follow-up.
- Works directly with patients/caregivers and an interdisciplinary team to expand access to health insurance, financial information, benefits, and valuable resources for patients.
- Interfaces with patients and families on an as-needed basis to address psychosocial needs.
- Maintains 45 credit hours (CEUs) for licensure renewal and certification.
- Completes continuing education coursework relevant to area of expertise.
- Under the direction of the physician, counsels families concerning the facilitation of the medical plan of treatment.
- Develops, prepares, and maintains individualized client care documentation with accuracy and timeliness, according to best policies and best practices.
- Respects and protects the patients' rights to confidentiality and privacy, and discloses information only for professional purposes which are in the patients' best interest with full consideration of their legal rights.
- Demonstrates initiative in continuously improving personal knowledge, skills, and work experience, as well as in improving departmental functioning.
- Formulates and maintains effective working relationships with peers and management, and functions as an effective team member.
- Receives constructive feedback in a positive, self-developmental manner, and is receptive and approachable.
- Performs other related duties incidental to the work described herein.
Position Requirements
Educational Requirements:
Mater's Degree of Social Work, required.
Licensure Requirements:
Current PA state licensure, required.
Experience Requirements:
- Demonstrates competence in providing basic social work services, including psychosocial counseling with individuals and families.
- Possesses the ability to understand medical terminology and communicates effectively with persons of diverse backgrounds.
