Position Details
Social Worker — Location: Phoenix, AZ. Department: PCH-MAIN | Social Work. Shift: Mon-Fri, Days, 8am-5pm. Category: Care Mgmt/Social Wrk/Case Mgr. Posting #: 966372. Employee Type: Full-Time.
Position Summary
This position provides clinical assessment, therapeutic interventions, care coordination, transition planning and emotional support to patients and families within their scope of practice. Participates as an interdisciplinary team member providing interventions including bio-psychosocial assessment, diagnosis, treatment planning, crisis interventions, coordination of care, bereavement support, and transition/discharge planning. Requires knowledge of family systems and interpersonal dynamics, and the ability to collaborate with medical staff, community resources, and outside authorities as needed. May provide individual, group, or family therapy for an assigned caseload and is responsible for implementing treatment assignments, managing open patient records, and coordinating with outside referral sources as applicable.
Responsibilities
- Psychosocial assessments and interventions, including bio-psychosocial status, mental health status, support systems, safety issues, risk assessment, and family systems.
- Identify therapeutic goals to improve family functioning, mental health, adherence, and self-sufficiency; collaborate with the interdisciplinary team on discharge planning.
- Documentation: complete clinical records in a timely manner and coordinate care through demographic updates, recommendations, and disposition planning; complete handoff documentation per Department procedures.
- Coordination of Care: develop and implement treatment plans and interventions; communicate with medical staff, nursing staff, case managers, peers, and families to facilitate care and continuity; participate in care conferences and rounds as needed; monitor transitions between outpatient and inpatient services.
- Monitor and review open patient records for policy compliance and service delivery per treatment plan; develop home contracts and related goals for patient care as appropriate.
Qualifications
Education: Master’s in Social Work from a CSWE-accredited program or an equivalent foreign degree as determined by the Foreign Equivalency Determination Service.
Experience: Child protection experience required; three years post-Master’s in a health or behavioral health setting preferred; pediatric social work experience preferred.
Special Skills: Excellent verbal and written communication; strong decision-making, problem-solving, and leadership skills; critical thinking; ability to assess symptoms and develop diagnostic impressions and treatment plans; moderate to expert computer skills.
Additional Requirements: Current State of Arizona Licensed Master in Social Work; CPR/First Aid and CPI training or ability to obtain at hire; DPS Fingerprint Clearance Card is preferred.
Certification / License: Licensed Master Social Worker – Required; Fingerprint Clearance Card – Required.
