Overview

Matrix Providers is hiring a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to provide inpatient coverage for day shift at Tucson VAMC, AZ. You will deliver high-impact care to Veterans, ensuring timely assessment and intervention in a fast-paced environment.

Employment Status: Full-Time

Schedule: Days: 7:30AM - 4:00PM or 8:00AM to 4:30PM. Weekdays. No OT.

Qualifications

  • Master of Science in Social Work (MSW) from a nationally accredited school
  • Active, unrestricted LMSW or LCSW
  • Current, full, and unrestricted social work license in any U.S. State, Territory, or the District of Columbia
  • Basic Life Support (BLS) certified by an American Heart Association (AHA)-approved vendor
  • NPI number
  • OIG Exclusion List Verification

Required Skills

  • Active SW license.
  • Minimum of two years of recent experience in an inpatient social work setting.
  • Background in mental health, substance abuse, and housing instability.
  • Demonstrated ability to work independently.
  • Strong interpersonal and assessment skills, especially in acute/emergent situations.
  • Familiarity with Tucson-area community resources is preferred.

Responsibilities

  • Conduct comprehensive biopsychosocial assessments.
  • Perform suicide risk, mental health, and crisis assessments and interventions.
  • Develop and implement care plans that address social determinants of health.
  • Document safety plans and interventions in electronic medical records.
  • Manage post-discharge case assignments and assist with discharge planning for Veterans, including referrals to rehabilitation, home health, long-term care, and palliative services.
  • Educate patients and families on care plans and advance directives to ensure understanding and compliance.
  • Coordinate inpatient psychiatric petitions and collaborate with Adult Protective Services on cases involving abuse, neglect, or housing instability.
  • Communicate critical patient information to interdisciplinary care teams.
  • Plan and facilitate family and interdisciplinary team meetings, including goals-of-care discussions.
  • Participate in interdisciplinary collaboration to ensure continuity of care and shared decision-making.
  • Maintain professional decorum and uphold VA customer service standards.
  • Respect patient rights, privacy, and individual dignity.
  • Accurately review and utilize electronic medical records to inform care decisions.
