Responsibilities

  • Completes psycho/social assessment promptly.
  • Provides social services (age and culturally-specific) to patients.
  • Assesses financial situation and acts on patient’s behalf for community, state, and national resources.
  • Identifies and resolves problems that impede discharge planning or the patient’s use of health care resources.
  • Collaborates with the care team to develop and revise a discharge plan as necessary throughout the patient’s stay and implement it to ensure a safe and reliable transition to the next level of care.
  • Provides education to patients and caregivers identified services.
  • Maintains a resource guide of area agencies, including HME, Home Care, Home Infusion, and Nursing Homes for patient choice and post-discharge services.
  • Maintains a resource guide of community resources, providers and partners.
  • Maintains criteria to prioritize high-risk patients.
  • Assists patients with advance directives.
  • Advocates for patient rights and protection.
  • Assists as a mediator with patient/family/facility conflicts or ethical dilemmas.
  • Serves as an authority in programs relating to community resources.
  • Timely processes, reports, and coordinates investigations of abuse, neglect, guardianship, and other cases to the appropriate advocacy, criminal, regulatory, and/or authority agency.
  • Participates in Quality and Performance Improvement activities.

Qualifications

  • Education: Bachelor’s Degree required. Education as a Generalist applying skills to all age groups. Master’s Degree in Social Work preferred.
  • Experience: Two years of clinical experience, preferably in acute care social work, care management, and discharge planning, required.
  • Licensure/Certification: Licensed in Kentucky by the State Board of Examiners of Social Work within 36 months from the date of employment required.
