If you are interested in this Hospice Social Worker position, please contact Lindsay at 586-710-7959 or LKOPASZ@ALINESTAFFING.com Hospice Social Worker Compensation • The pay for this position is $31.92 per hour • Benefits are available to full-time employees after 90 days of employment • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates Hospice Social Worker Highlights • This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs • The required schedule is Monday through Friday, 8:00am to 5:00pm (no weekends) • Travel required within Washington, Ramsey, Dakota, and parts of Hennepin Counties – mileage reimbursed at $0.50/mile Hospice Social Worker Responsibilities • Support individuals, families, and groups in various settings such as hospitals, schools, detention centers, and treatment facilities • Assist clients dealing with chronic and temporary challenges such as homelessness, substance abuse, unemployment, and domestic violence • Prevent, assess, and evaluate psychosocial conditions; formulate and implement plans of action • Provide interventions for mental, emotional, developmental, and behavioral disorders • Engage in case management, advocacy, counseling, consultation, administration, and teaching • Administer psychosocial functioning tests and communicate with involved agencies on behalf of clients • Conduct individual, group, and/or family therapy as needed • Participate in staff rounds and utilization review as part of an interdisciplinary team • Coordinate discharge planning and follow-up tasks • Document client activities, monitor progress, and maintain updated case records • Provide emergency assessments in collaboration with psychiatrist Hospice Social Worker Requirements • Master’s Degree in Social Work (MSW) – required • Supervised clinical experience – required • Valid Driver’s License and reliable transportation – required • Attendance is mandatory for the first 90 days Hospice Social Worker Preferred Qualifications • Hospice experience preferred • Experience with HCHB (Homecare Homebase) is helpful but not required If you think this Hospice Social Worker position is a good fit for you, please reach out to me – feel free to call, e-mail, or apply to this posting!
