The Behavioral Health Division’s K-12 Case Management Team, in collaboration with Multnomah County Schools, provides highly complex case management and prevention services to kindergarten through twelfth grade students and their families, with a focus on increasing attendance and reducing emotional / behavioral issues that interfere with educational success.

The Case Manager Senior - KSA African American (AA) provides specialized, short-term, intensive case management / coordination of services for children who have complex conditions and circumstances and are experiencing high utilization or underutilization of health and social service resources across multiple systems. Individuals may be at risk of losing school placement, abuse, neglect, homelessness or loss of community placement. This position assists families with connecting to student mental health resources. The position also provides advocacy, assists with navigation of local, state and federal systems, and links families to a variety of resources, including food, transportation, clothing, housing, legal, financial, and culturally specific services.

Knowledge, Skills and Abilities (KSA): Advanced knowledge of and experience working with the Black and African American community. Understands cultural, racial and ethnic factors, including systemic advantage or disadvantage; acculturation, intergenerational immigrant family challenges, effects of trauma during immigration, war, separation from family and/or primary supports, detention, refugee camps, etc.; generational trauma, and any factors related to non-dominant culture affecting mental health and treatment. Specifically knows the African American community, historical and current barriers to mental health services and has knowledge of the traditions, values and customs specific to this population.

As the Case Manager Senior - KSA AA, other duties will include:

  • Complex Case Management/Coordination of Services

    • Provide highly complex case management and prevention services to kindergarten through twelfth grade students and their families, with a focus on increasing attendance and reducing emotional / behavioral issues that interfere with educational success.
    • Provide specialized, short-term, intensive case management / coordination of services for children who have complex conditions and circumstances and are experiencing high utilization or underutilization of health and social service resources across local, state and federal systems.
    • For children that do not meet target criteria, assist families with connecting to community resources and referrals for mental health, developmental disability and social service needs in collaboration and partnership with multiple systems.
    • Provide initial intake and engagement with student and their family.
    • Administer a strengths / needs assessment tool to develop targeted interventions to assess the child's history of emotional well-being, mental health and substance use, family / relationships / cultural identity, social / recreational, risk and safety concerns, family legal system involvement, educational / vocational, health / medical, home / a place to live / financial, trauma experience, or other services / supports and family strengths.
    • Assessments may include interviewing individuals and their support network; consulting with mental health / medical or other professionals as needed; reviewing, interpreting and collecting mental health/medical and social information from various information sources.
    • Discuss service options with client and family/support network, offering various options and providing decision support by providing client and their support network with as much information as possible about the limitations and strengths of each option.
    • Work with clients and their support network, establish a plan for services, determine best methods to implement the plan and provide support and resources to clients and providers as needed to implement the plan and access benefits.
    • Coordinate intensive and complex case management and coordination of services with pertinent agencies and individuals, such as: doctors, hospitals, health/mental health clinics, attorneys, guardians, natural support networks, etc.
    • Assess barriers to school attendance which may include language, culture, food, housing, clothing, refugee or immigration status, learning disabilities, income, parental experience of education systems or mental health.
  • Outreach and Coordination

    • Provide outreach and coordination with social service networks.
    • Establish and maintain working relationships with community partners, school counselors, physicians and mental health providers who increase client access to additional supports.
    • Serve as a point person to address barriers by referring and assisting with acquiring resources, connecting families with the school and communicating with the school around barriers the family is experiencing.

Minimum Qualifications (REQUIRED):

Education/Training: Equivalent to Bachelor's Degree from an accredited college or university with major course work in social science, social work, sociology, public health, community health, behavioral science, or related field; AND

Certification: Must meet the standards of Qualified Mental Health Associate - (QMHA I) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at MHACBO website. Current QMHA I (Required)

Preferred Qualifications/Transferable Skills: You do not need to have the following preferred qualifications/ transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates.

  • 1-2 years experience providing case management services
  • Knowledge of issues faced by those who experience behavioral health challenges and a basic understanding of the full continuum of care for those in need of services.
  • Lead with race through actions and advocacy with internal programs, with community partners and across departments throughout the county.
  • Demonstrate the ability to flex style when faced with myriad dimensions of culture in order to be effective across cultural context.
  • Respect and appreciation for ethnic and cultural diversity.
  • Ability to collaborate and build relationships to achieve positive work outcomes.
  • Ability to maintain a safe and healthful workplace.
  • High degree of resilience, is outcome driven and can thrive in an environment of rapid change while effectively managing pressure in an effective and professional manner.
