Overview

Ennoble Care is a mobile primary care, palliative care, and hospice service provider serving patients in multiple states. Clinicians visit patients at home, providing continuum of care for those with chronic conditions and limited mobility. Ennoble Care offers programs including remote patient monitoring, behavioral health management, and chronic care management. Responsibilities

  • Provide pastoral/spiritual care to patients and/or families.
  • Visit and minister with patients; collaborate with the Clinical Director and others to develop a spiritual care continuum of care for hospice patients and families.
  • Conduct spiritual assessments and develop care plans addressing physical, emotional, and spiritual needs and goals.
  • Provide bedside pastoral support, and, as requested, support for funerals and memorials.
  • Document visits with patients per company policy and compliance requirements.
  • Function as a member of the interdisciplinary team and participate in staff meetings and in-services.
  • Organize company-sponsored memorial services as appropriate and provide emotional/spiritual support to staff as needed.
  • Initiate and maintain communication with area inter-faith clergy and facilitate spiritual care bridging between patients, families, and clergy for end-of-life support.
  • Educate staff about religious needs and issues affecting patient care and treatment.
  • Collaborate with the interdisciplinary team to ensure holistic patient care and contribute value-added activities for the well-being of clients, patients, families, and staff.

Education/Qualifications

  • Master’s of Divinity or equivalent in theological studies required.
  • At least four units of Clinical Pastoral Education (CPE). Board Certified as a Chaplain or eligible for Board Certification under ACPE or CPSP.
  • Excellent communication and pastoral skills; ability to support patients and families experiencing pain, suffering, death, and grief; strong organizational and decision-making abilities.
  • Computer literacy and ability to perform charting in the Company’s EMR system.

Licensure

  • Certification or eligibility for certification through CPSP or ACPE.

Experience

  • Minimum of two years of pastoral experience in a healthcare setting; hospice experience is desirable with demonstrated empathy and ability to support patients/families of inter-faith traditions.

  • Strong interpersonal skills and the ability to work with diverse ages, lifestyles, cultures, socio-economic backgrounds, and religious beliefs.
  • Professional knowledge to develop and maintain collaborative relationships with physicians, nurses, social workers, and other team members.
  • Ability to work independently, take initiative, and be accountable; maintain adequate physical and emotional health.
  • Ecclesiastical endorsement or ordination is desirable.
