Description
LTC Social Worker needed for part-time, temporary position in Philadelphia County.
Manages the social work function from admission through discharge, including room changes, care transitions, and discharge planning with all necessary services and equipment. Collaborates with mental health providers, makes referrals, and leads interdisciplinary care plan meetings with families. Initiates psychosocial interventions, palliative, and hospice referrals, and participates in support groups and Resident/Family Councils as needed. Acts as the facility's Grievance Officer and ensures all services respect resident rights and comply with regulatory standards. Maintains knowledge of insurance models, including medical assistance applications.
Completes all required documentation, including admission assessments, psychosocial assessments, MDS, and other records promptly and in line with department policy and State and Federal guidelines. Educates residents, families, and staff on advanced directives and provides psychosocial and bereavement support to ease adjustment to the community.
Position Requirements
BSW and a minimum of 1 year of social work experience, preferably in long-term care or assisted living, with knowledge of geriatric populations and dementia.
