Responsibilities

  • Administrative Functions - Assist the Director of Nursing Services in directing the day to day functions of the nursing activities in accordance with current rules, regulations, and guidelines that govern the long term care facility. Participate in developing, maintaining, and updating written policies and procedures that govern the day to day functions of the nursing service department. Ensure reference material (i.e., PDRs, regulations, professional standards of practice) is current. Recommend written material to assist the nursing service department in meeting residents\' day to day needs. Ensure the Nursing Service Procedures Manual is current and reflects day to day nursing procedures. Ensure all nursing service personnel comply with the procedures in the Nursing Service Procedures Manual. Make written and oral reports/recommendations to the Director as necessary concerning the operation of the nursing service department. Periodically review the department\'s policies, procedure manuals, and job descriptions; make revisions as needed. Assist in coordinating nursing services with other resident services to ensure continuity of the residents\' total regimen of care. Ensure nursing service personnel are aligned with their job descriptions. Participate in the development, maintenance and implementation of the facility\'s quality assurance program for the nursing service department. Participate in facility surveys (inspections) by authorized government agencies as requested. Periodically review the resident\'s written discharge plan and assist in updating it as required. Assist the Director in planning the nursing services portion of the resident\'s discharge plan. Meet with the nursing staff and support personnel to plan department services, programs, and activities. Admit, transfer, and discharge residents as required. Complete accident/incident reports as necessary. Interpret the department\'s policies and procedures to personnel, residents, visitors, and government agencies as required. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary. Maintain information security by safeguarding user IDs/passwords and reporting violations, and protect resident health information as required. Report any unauthorized access attempts.
  • Committee Functions - Attend facility committee meetings (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment) as required. Prepare written and/or oral reports of nursing service programs to committees. Provide information to the Quality Assurance and Assessment Committee as requested. Participate in developing the agenda for the Care Plan Committee meetings and in reviewing discharge plans. Implement committee recommendations as instructed by the Director.
  • Personnel Functions - Ensure staffing levels of licensed nurses and CNAs to provide quality care. Develop work assignments and delegate administrative authority to Charge Nurses. Ensure all CNAs are enrolled in or graduated from an approved training program. Supervise daily rounds, report findings to the Director. Monitor absenteeism and address problem areas. Ensure adherence to policies including dress code. Review complaints and grievances. Build good interdepartmental relationships to support resident needs. Foster a warm, calm unit atmosphere. Lead regular shift meetings to address problem areas and improve services. Participate in evaluations, staffing decisions, and disciplinary actions as needed. Ensure compliance with policies and fair disciplinary actions regardless of protected characteristics. Report occupational exposures and suspected fraud as required. Ensure secure workstations and password protection per policy.
  • Nursing Care Functions - Inform staff of new admissions; ensure rooms are prepared and residents are greeted and escorted as needed. Coordinate with physicians for timely notes and orders. Plan with residents, families, and physicians for care plans; notify changes in condition to the attending physician and next of kin as appropriate. Conduct daily resident visits to assess status. Ensure direct nursing care is provided by licensed personnel or qualified aides. Review nursing notes for accuracy and consistency with resident wishes. Perform direct nursing care when needed. Initiate and monitor procedures (e.g., IVs, lab specimens, vital signs). Manage admissions, transfers, and discharges; report problems to the Director. Assist in corrective actions and keep the Director informed. Monitor medication administration and charting; ensure accuracy and adherence to policies. Inspect care areas daily and maintain cleanliness and safety. Ensure residents unable to call for help are checked regularly. Communicate with residents and families as necessary. Arrange transportation for discharges and notify families of resident death; handle funeral home arrangements as requested.
  • Staff Development - Develop and participate in in-service training and continuing education to maintain licensure and keep staff informed on safety, HIPAA, infection control, etc. Assist in clinical supervision for nurse aide trainees.
  • Safety and Sanitation - Assist in establishing safety standards; ensure exposure control training for tasks with potential exposure. Maintain PPE supplies and enforce safety and hand hygiene. Conduct drills and participate in infection control programs. Ensure proper handling of hazardous conditions and equipment. Ensure MSDSs are on file and that staff use safety equipment when lifting or moving residents. Report missing or illegible labels and ensure proper labeling of hazardous chemicals. Ensure cleanliness and sanitation of work areas and resident rooms. Maintain hand hygiene and safety compliance across the department.
  • Equipment and Supply Functions - Advise on equipment and supply needs; maintain adequate stock. Assist in preventive maintenance programs and safe operation of equipment. Ensure only trained staff operate equipment and that supplies are used efficiently. Maintain MSDS documentation for hazardous chemicals.
  • Care Plan and Assessment Functions - Contribute to preliminary and comprehensive resident assessments and care plans. Encourage resident participation in care planning. Review and ensure care plans guide daily care and that staff refer to the plan before care. Assist in planning, scheduling, and revising the MDS (including RAPs and Triggers). Ensure treatments align with care plans and resident wishes.
  • Budget and Planning Functions - Stay informed on economic conditions; recommend adjustments to sustain quality care. Inform on equipment needs and report fraud related to false billings and other improper activities.

Qualifications

  • Education and Experience - Nursing degree from an accredited college/university. Training in rehabilitative/restorative nursing. At least 2 years of experience, preferably in a long-term care facility.
  • Licenses and Certifications - Current, unencumbered RN license in this state. Current CPR certification. Knowledge/experience with PCC preferred. Maintain ongoing continuing education/licensing and good standing with the State Board of Nursing.
  • Language and Communication - English language proficiency (reading, writing, speaking). Ability to read technical procedures and policy manuals; effectively present information.
  • Mathematical and Reasoning Skills - Able to apply fractions, percentages, ratios, and proportions. Able to make independent decisions and solve practical problems; communicate tactfully with residents, families, and staff.
  • Leadership - Demonstrated leadership and supervisory ability; capable of planning, organizing, implementing, and interpreting programs and procedures to provide quality care. Team-oriented and able to collaborate across departments.
  • Physical and Other Requirements - Ability to move intermittently, speak and write English clearly, cope with mental/emotional stress, see/hear adequately, and meet health requirements. Must be able to relate to ill, disabled residents and staff. Ability to push/pull/move at least 25 pounds and carry 50 feet; may assist in evacuations.

Certificates, Licenses, Registrations

  • Current RN license, CPR certification, and continuing education compliance; PCC knowledge preferred.

  • Office and nursing service areas; frequent interruptions; contact with residents, families, staff, and government agencies; possible hostile or emotionally upset individuals. May require after-hours work, weekends, holidays, and emergency call-backs. Exposure to various health hazards and chemicals; frequent hand hygiene; potential for infectious waste exposure.
