Responsibilities

  • Administrative Functions
    • Assist the Director of Nursing Services in directing the day to day functions of the nursing activities in accordance with current rules, regulations, and guidelines that govern the long term care facility.
    • Participate in developing, maintaining, and updating written policies and procedures that govern the day to day functions of the nursing service department.
    • Ensure that reference material maintained at the nurses' stations is current.
    • Recommend written material that will assist the nursing service department in meeting the day to day needs of the resident.
    • Ensure that the Nursing Service Procedures Manual is current and reflects the day to day nursing procedures performed in this facility.
    • Ensure that all nursing service personnel comply with the procedures set forth in the Nursing Service Procedures Manual.
    • Make written and oral reports/recommendations to the Director as necessary/required concerning the operation of the nursing service department.
    • Periodically review the department's policies, procedure manuals, and job descriptions; make recommendations for revisions to the Director.
    • Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of the residents' total regimen of care.
    • Ensure that all nursing service personnel are in compliance with their respective job descriptions.
    • Participate in the development, maintenance and implementation of the facility's quality assurance program for the nursing service department.
    • Participate in facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services.
    • Periodically review the resident's written discharge plan and participate in updating it as required; assist in planning the nursing services portion of the resident's discharge plan as necessary.
    • Meet with the nursing staff, as well as support personnel, in planning the department's services, programs, and activities.
    • Admit, transfer, and discharge residents as required; complete accident/incident reports as necessary.
    • Interpret the department's policies and procedures to personnel, residents, visitors, and government agencies as required.
    • Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
    • Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Administrator.
  • Committee Functions
    • Attend various committee meetings of the facility as required (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment).
    • Assist in preparing written and/or oral reports of the nursing service programs and activities to submit to such committee as directed.
    • Provide information to the Quality Assurance and Assessment Committee as requested.
    • Participate in developing the agenda for the Care Plan Committee meetings as necessary.
    • Participate in regularly scheduled reviews of discharge plans and make such plans available to the Care Plan Committee as required.
    • Implement recommendations from established committees as instructed by the Director.
  • Personnel Functions
    • Ensure that a sufficient number of licensed practical and/or registered nurses are available for each shift to maintain quality care.
    • Ensure that a sufficient number of certified nursing assistants are available for each shift to provide routine nursing care to residents.
    • Develop work assignments and/or assist in completing and performing such tasks; delegate to each Charge Nurse the administrative authority, responsibility, and accountability necessary to perform assigned duties.
    • Ensure that all nursing assistants are enrolled in or have graduated from an approved nursing assistant training program; ensure nurse aide trainees are under direct supervision of a licensed nurse.
    • Make daily rounds of the nursing service department to ensure that all staff are performing their duties in accordance with acceptable nursing standards; report findings to the Director.
    • Monitor absenteeism to ensure adequate staffing; report problem areas to the Director.
    • Ensure that policies including dress codes are followed; review complaints and grievances filed by department personnel; report as required.
    • Develop and maintain a good working rapport with inter department personnel to ensure nursing services meet resident needs; create a warm, calm environment in the unit.
    • Meet with shifts regularly to identify problem areas and improve services; participate in evaluations and staffing decisions; advise on disciplinary actions when appropriate.
    • Ensure compliance with applicable laws and regulations; report occupational exposures and incidents of fraud per facility policy; ensure computer workstations are securely logged off when unattended.
  • Nursing Care Functions
    • Inform nursing staff of new admissions, expected arrival times, and room assignments; prepare rooms for new admissions; greet and escort new residents as needed.
    • Make rounds with physicians as necessary; encourage timely documentation of progress notes and orders; inform the Director if physician visits are not timely.
    • Consult with residents, families, and physicians in planning care; notify attending physicians and next of kin when there is a change in condition.
    • Visit residents daily to observe physical and emotional status; ensure direct nursing care is provided by licensed staff or trained aides; review nurses' notes for accuracy and alignment with resident wishes.
    • Provide direct nursing care as needed; perform procedures and collect specimens as ordered; admit, transfer, and discharge residents as necessary.
    • Report problems to the Director; assist in corrective actions; keep the Director informed through written or oral reports.
    • Assist the Charge Nurse in monitoring seriously ill residents; authorize use of restraints only when permitted by policy; ensure medication procedures are followed; review medication cards and ensure safe administration.
    • Inspect nursing service treatment areas daily; ensure cleanliness and safety; check residents unable to call for help frequently.
    • Meet with residents or family members as necessary; assist with discharge transportation; inform families of resident death and arrange funeral contact as requested.
  • Staff Development
    • Develop, plan, conduct and schedule in-service training to ensure a well educated nursing service department.
    • Assist in annual facility in-service training programs (OSHA, TB, HIPAA, Abuse Prevention, Safety, Infection Control, etc.).
    • Assist the Director in planning clinical supervision for nurse aide trainees; participate in continuing education to maintain licensure.
  • Safety and Sanitation
    • Assist in developing safety standards; identify and classify tasks with exposure to blood or body fluids; ensure in-service training for exposure control.
    • Maintain adequate PPE; ensure proper use and disposal; enforce hand hygiene; participate in fire safety and disaster drills.
    • Support infection control programs for monitoring communicable diseases; follow isolation precautions; report hazardous conditions or equipment; ensure MSDSs are on file.
    • Ensure staff wear safety equipment when lifting or moving residents; maintain clean and sanitary work areas; ensure proper handling of hazardous chemicals.
  • Equipment and Supply Functions
    • Recommend equipment and supply needs; maintain stock levels of medications, supplies, and equipment.
    • Coordinate preventive maintenance with Maintenance Director; ensure safe operation of all nursing service equipment; ensure only trained personnel operate equipment; minimize waste; maintain MSDSs for hazardous chemicals.
  • Care Plan and Assessment Functions
    • Participate in developing preliminary and comprehensive assessments and plans of care for each resident; involve residents in developing and reviewing care plans.
    • Review care plans for goals and revisions; ensure staff refer to care plans before delivering daily care; review nurses notes for adherence to care plans.
    • Assist in planning, scheduling, and revising the MDS, RAPs and Triggers; ensure care plans align with resident treatments and wishes.
  • Budget and Planning Functions
    • Keep informed on economic conditions and propose adjustments to ensure sustained quality care; advise on equipment needs; report fraud related to billing as appropriate.
  • Supervisory Requirements
    • Assist with overall supervision and management of the nursing staff; as an RN, you are delegated administrative authority and accountability for assigned duties.

Qualifications

  • Education and/or Experience
    • Must possess a Nursing Degree from an accredited college or university; training in rehabilitative and restorative nursing practices; two years or more experience in a long term care facility preferred.
  • Language Skills
    • Must be able to read, write, speak, and understand English; ability to read technical procedures and policy manuals; ability to present information and respond to questions from managers and employees.
  • Mathematical Skills
    • Ability to apply fractions, percentages, ratios and proportions to practical situations.
  • Reasoning Ability
    • Ability to make independent decisions when warranted; solve practical problems; ability to deal tactfully with personnel, residents, families, visitors, government agencies and the public; leadership and supervisory ability; ability to plan, organize, develop and interpret programs and procedures for quality care.
  • Certificates, Licenses, Registrations
    • Current, unencumbered active RN license in the state; current CPR certification; PCC knowledge preferred; maintain continuing education/licensing; remain in good standing with the State Board of Nursing.
  • Physical Demands
    • Move intermittently; speak and write English; cope with mental and emotional stress; see and hear adequately; function independently; pass health requirements; relate to ill and elderly residents; lift at least 25 pounds and move 50 feet; may assist in evacuations.
  • Work Environment
    • Work in office and nursing service areas; frequent interruptions; interact with residents, families, staff, and government personnel; may work beyond normal hours; may be exposed to infectious waste and hazardous chemicals; participate in educational programs.
