Role Responsibilities Conduct evaluations of patients in inpatient or outpatient settings and create personalized treatment plans Implement physical therapy interventions focusing on mobility, strength, balance, and overall functionality Collaborate with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation specialists to achieve optimal outcomes Instruct patients and family members about rehabilitation goals, discharge planning, and home exercise programs Maintain accurate documentation on evaluations, treatments, and patient progress following facility and regulatory standards Engage in care team meetings and interdisciplinary rounds to ensure seamless coordination of care Qualifications
Doctorate in Physical Therapy (DPT) or a related degree from an accredited institution Certifications
State licensure as a
Physical Therapist CPR certification strongly preferred Additional certifications in specialized areas (e.g., neurological, geriatric) are appreciated but not mandatory Essential Skills
Prior experience in a similar setting is greatly preferred Proficient in treating patients with complex medical and mobility challenges Excellent communication and teamwork abilities Comfortable with digital charting and clinical documentation processes Benefits You Will Receive
