Position Title: Case Manager Supervisor Reports To: Director of Social Services Department: Social Services Employment Status: Full-Time FLSA Status: Non-Exempt Position Status: Essential
GENERAL JOB DESCRIPTION
The Case Manager Supervisor is responsible for overseeing the team of Case Managers and ensuring the handling of clients' cases and carrying out all responsibilities from intake to move out. They will responsible for ensuring that Case Managers act as the liaison for management and communicate clients' progress, as well as, any case concerns.
DUTIES AND RESPONSIBLITIES Oversees Case Management for the entire facility Responsible for ensuring that the responsibilities of Case Managers are being fulfilled Must provide active and consistent feedback to Case Managers regarding performance and progress Ensures that client files, ILPS, Progress notes, room inspections, etc. are in compliance Helps assist with performance evaluations Must provide job coaching to Case Managers as necessary. In the event of an emergency, Essential Employees are required to report to work as scheduled Duties and responsibilities subject to change based upon assessment of company needs SKILLS AND QUALIFICATION
Bachelor's Degree in Psychology, Social Work, Sociology, Mental Health, Nursing, Health and Human Services or Education Services required Must have a minimum two years' experience working within the Social Services Field Must have interpersonal skills Must be able to communicate effectively (verbally and in writing) Bilingual English/Spanish is preferred Experience working with the Homeless Population Experience with the CARES system Job Type: Full-time
Pay: $65,000.00 per year
