Job Summary

Odyssey House Louisiana, Inc. is a seeking a motivated and compassionate full-time Family Practitioner to work Monday-Friday 8am-5pm. Hybrid, fully onboarded, at the Community Health Center. The Nurse Practitioner will participate in day-to-day clinic services as a direct care provider. This position will manage Quality Improvement/Quality Assurance reports, as part of the Best Practices of the clinic and other programs, that reflect participation in coordinating and providing patient care.

DUTIES & RESPONSIBILITES

It is the duty and responsibilityof the Nurse Practitioner to:

provide direct care to patients within the Scopeof Practice for which the provider is licensed; provide direct care to clients/patients tosupport the agency'sdepartments i.e. Community Health Center (CHC), Detox, Residential, Transitions,etc....; manage patient care under Best Practices, UDSrecommendations, and as prescribed to maintain accreditation; conduct thorough documentation of visits andmedical orders to support billing charges; complete billing components of the visit tooptimize revenue within dictated time frames; oversee care delivered by medical support staffby providing feedback on immediate care activities and reporting to directsupervisor on concerns that expose the agency or endanger a patient's healthoutcomes; participate in webinars and conference calls; implement and manage Best Practices initiated byQuality Improvement/Quality Assurance staff; identify and prioritize medical decision makingwith a fiscally minded approach to care; coordinate and supervise the completion of dailytasks related to direct patient care on matters related to Health Outcomes (i.e.referrals, lab orders, lab review, and medical records review); organize and implement workflow strategies tosupport provider visits, billing, and patient care as part of the feedback to CHCleadership during One on Ones and other meetings; coordinate with CHC leadership in the implementationof Quality Insurance measures to ensure data integrity and outcomes are met; ensure support staff document all pertinent health-relatedinformation in the client's electronic medical chart as instructed and as part ofstandards of practice designed to improve patient care, support billing, andmeet Quality Improvement goals; observe Occupational Safety and HealthAdministration during all elements of daily operations, including but notlimited to phlebotomy, in-house testing, medical interventions, medical waste,and managing infection control; observe Health Insurance Portability andAccountability Act compliance; meet regularly for One on Ones with the LeadNurse Practitioner; participate in regularly scheduled meetings inthe CHC; timely respond to the CHC Chief Medical Officer,Lead Nurse Practitioner, and Clinic Supervisors; ensure timely completion of all assignedadministrative reports; train and orient new staff, providers, orcontractors to the clinical practice as needed i.e., EMR, HIV testingprotocols, etc.; keep licensure current and meets credentialrequirements; update the CHC Chief Medical Officer, Lead NursePractitioner, and Clinic Supervisors on the practice and trends, changes,alerts, concerns, and all matters which impact patient care, services rendered,or billing; attend continuing education programs thatprovide updated information on health topics pertinent to patient care andeffective clinic practice; participate in the implementation and support ofQuality Improvement/Quality Assurance initiatives; and perform other duties as assigned. PHYSICAL DEMANDS

Must be able to sit, stand, and walk for long periods up to 40%of the time. Must be able to bend, lift, and twist 10% of the time. May beasked to lift 20 pounds.

WORK ENVIRONMENT

Work is normally performed inside of the premises, which isclimate controlled. Risk of exposure to sick patients is consistent with thatwith any clinic setting offering primary care services. The employee will berequired to travel to other facilities up to 20% of the time and must bewilling to work various hours to meet operational demands.

MINIMUM QUALIFICATIONS

Minimum qualifications include the following:

Master's Degree in Nursing Science Active, unencumbered LA NP license withprescriptive authority Current good standing with regulating NationalCertifying Board At least 1 year of clinical experience At least 1 year of Electronic Health Recordsexperience CPRcertification Proficient in MS Office 365 Excellent communication skills Ability to exercise patience, approachability,and consistency in a stress high-stress environment PREFERRED QUALIFICATIONS

Preferred qualifications include the following:

At least 1 year of FQHC and medical home model experience Suboxone certification
