Description Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting tailored treatment plans to meet individual needs Implement physical therapy interventions focusing on enhancing mobility, strength, balance, and overall function Work closely with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal patient outcomes Guide and educate patients and their families about therapeutic objectives, home exercise regimens, and discharge planning Meticulously document evaluations, treatment plans, and patient progress in line with organizational and regulatory standards Engage in care team meetings and interdisciplinary discussions to maintain coherence in patient care Education

Doctorate in Physical Therapy (DPT) or equivalent from an accredited program Certifications

State licensure as a

PT / Physical Therapy Specialist CPR certification is preferred Additional specialized certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory Skills

Prior experience in a similar role is highly preferred Proven ability to assist patients with varying complexities in medical and mobility issues Exceptional communication and team collaboration skills Proficient in digital charting and clinical documentation systems Benefits
