Description Conduct thorough evaluations of patients in either inpatient or outpatient settings and develop custom-tailored treatment plans Implement physical therapy interventions to enhance mobility, strength, balance, and overall function Work collaboratively with a team of nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes Educate patients and their families on therapeutic goals, plan for discharge, and provide guidance on at-home exercises Maintain documentation of evaluations, treatment sessions, and patient progress per facility and regulatory standards Engage in interdisciplinary conferences and care team rounds to ensure streamlined and effective patient care Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution Certifications
State licensure as a
Physical Therapist CPR certification preferred Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not required Skills
Previous experience is strongly preferred Ability to care for patients with complicated medical and mobility challenges Excellent communication and teamwork skills Proficiency with digital charting and clinical documentation systems Benefits
