Kern River Transitional Care Center is seeking an experienced and dynamic

Admissions Director

to manage and oversee our admissions process. This role ensures a smooth, professional, and compassionate experience for residents, families, referral partners, and staff. The Admissions Director plays a key role in maintaining census goals, coordinating with clinical and administrative leaders, and building strong community relationships to support our mission of providing excellent care. Key Responsibilities Manage the admissions process from inquiry to intake, ensuring a positive and professional experience. Plan, direct, and monitor the daily operations of the Admissions Department to meet census goals. Screen and admit patients by verifying admission criteria, completing admission and financial forms, and confirming insurance benefit coverage. Collaborate with the

Director of Nursing, Case Managers, and facility leadership

to ensure the appropriateness and timeliness of admissions. Maintain accurate records of

bed availability

and oversee patient bed assignments. Build and maintain strong relationships with

physicians, social workers, discharge planners, and referral sources

to maximize referrals. Participate in daily stand-up meetings to ensure effective communication across departments. Coordinate physical, social, emotional, and support services, including transportation when needed. Educate referral sources and community partners about facility services through

outreach events, health fairs, and promotional activities. Ensure confidentiality of all resident care information, including PHI, and compliance with facility policies. Provide leadership and supervision to admissions staff to ensure departmental efficiency. Qualifications Minimum: High School Diploma or GED. Preferred:

Bachelor's Degree in Healthcare or related field. Two years of admissions experience in healthcare preferred. LVN or RN license preferred. Proficiency in

Microsoft Suite

required. Strong organizational, leadership, and communication skills. Ability to solve problems and make decisions in fast-paced, variable situations. Physical & Work Environment Requirements Ability to lift/move up to 25 lbs occasionally. Frequent sitting, standing, walking, reading, and computer use. May assist in resident evacuation during emergencies. Work environment typically
