Description Assess patients in inpatient or outpatient environments and develop tailored treatment plans Implement physical therapy interventions targeting mobility, strength, balance, and function Collaborate effectively with nurses, physicians, case managers, and other rehab professionals to achieve optimal results Instruct patients and their families on therapeutic goals, discharge planning, and home exercises Document evaluations, treatments, and patient progress according to facility and regulatory guidelines Engage in care team rounds and interdisciplinary conferences to maintain coordinated care Education

Doctorate in Physical Therapy (DPT) or equivalent degree from a regionally accredited program Certifications

Current state licensure as a

Physical Therapist Current CPR certification is preferred Specialized certifications (neurological, geriatric) are advantageous but not mandatory Skills

Previous experience in a similar setting is strongly preferred Capable of treating patients with complex medical and mobility challenges Excellent communication and teamwork abilities Proficient with digital charting and clinical documentation systems Benefits
