Description Conduct thorough evaluations of patients in inpatient or outpatient settings and develop personalized treatment plans Implement physical therapy techniques that enhance mobility, strength, balance, and overall functionality Partner with nurses, physicians, case managers, and various rehabilitation experts to ensure optimal patient outcomes Guide patients and their families on therapeutic objectives, discharge procedures, and necessary at-home exercises Maintain accurate and comprehensive documentation of evaluations, treatments, and patient progress in line with facility and regulatory protocols Engage in care team rounds and interdisciplinary meetings for seamless care management Education

Doctorate in Physical Therapy (DPT) or similar qualification from an accredited institution Certifications

State licensure as a

Physical Therapist CPR certification is preferred Specialized certifications (e.g., neurological or geriatric care) are advantageous but not mandatory Skills

Prior experience is highly advantageous Ability to manage patients dealing with complex medical and mobility challenges Excellent communication and collaborative skills Familiarity with digital charting and clinical record management Benefits
