Description Conduct evaluations in both inpatient and outpatient settings while crafting tailored treatment plans Deliver specialized physical therapy interventions targeting mobility, strength, stability, and functional recovery Collaborate with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation experts to optimize patient outcomes Provide education to patients and their families regarding therapeutic goals, discharge plans, and home care exercises Maintain comprehensive documentation of evaluations, treatments, and progress following regulatory and facility requirements Participate in interdisciplinary care rounds and conferences to ensure seamless patient-centered care Education

Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution Certifications

Active state licensure as a

Physical Therapist CPR certification preferred Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory Skills

Previous experience in the field is highly preferred Ability to work with patients with multifaceted medical and mobility needs Excellent communication and teamwork skills Proficient in digital documentation and clinical record-keeping systems Benefits
