Description Assess and evaluate patients in both inpatient and outpatient settings, crafting tailored treatment plans that address individual needs. Execute targeted physical therapy interventions focused on improving mobility, enhancing strength, and ensuring optimal function. Work collaboratively with an interdisciplinary team-including nurses, physicians, case managers, and other rehabilitation professionals-to maximize patient outcomes. Provide education to patients and their families about therapeutic goals, discharge processes, and prescribed home exercise routines. Maintain meticulous documentation of patient evaluations, therapy sessions, and progress in accordance with all facility and regulatory requirements. Engage in care team meetings and interdisciplinary discussions to ensure a cohesive approach to patient care. Education

Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution. Certifications

Valid state licensure as a

Physical Therapist . CPR certification preferred. Specialty certifications (e.g., neurological, geriatric) are an advantage but not mandatory. Skills

Prior experience is a plus. Ability to effectively treat patients presenting complex medical and mobility challenges. Excellent communication and teamwork capabilities. Proficient with digital charting and clinical documentation systems. Benefits
