This position reports to the Breathitt County Guardianship office andmay be responsible for covering multiple counties within the service region, including Breathitt, Lee, Wolfe,Owsley, Leslie, Perry, Magoffin, Knott, Letcher, Floyd, Pike, Martin, Johnson, and the surrounding areas.
Description
The Kentucky Department for Community Based Services (DCBS) is a multi-faceted agency that provides servicesand programs to enhance the self-sufficiency of families; improve safety and permanency for children andvulnerable adults; and engage families and community partners in a collaborative decision-making process. The Division of Guardianship makes personal and financial decisions for individuals adjudicated dependent andappointed by the District Courts to the Cabinet for Health and Family Services. The division providesindividualized decisions based on the best interests, most appropriate, and least restrictive alternatives forthose appointed to guardianship. The department works with a very diverse population, community resources, thecourt system,community partners, and family members of individuals appointed to guardianship and other Cabinet anddepartment staff in a collaborative relationship. The Department is looking for an individual with the passionto help others.
Responsibilities include but are not limited to the following: Provides entry level professional social work services to individuals with disabilities who have beenassigned to the Cabinet for Health and Family Services under state guardianship or conservatorship throughthe assessment of client needs and the provision of social services. Perform other duties as required. Under direct supervision, provides guardianship services for disabled persons to include making all medical,personal and/or financial decisions in all aspects of the client's life according to the order of the court. Under direct supervision, provides visits to clients to assess health, safety and welfare and livingconditions and assists them with access to community resources to include place of residence, hospitals, andnursing homes where there may be exposure to infectious diseases. May assist with moving personal belongingsto include lifting heavy objects. Enters data in the Kentucky Guardianship Fiduciary Information System, including initial adjudication andintake of the individual under guardianship, and continuing events related to each client served. Maintainsclient's records. Assists in the preparation of court reports by gathering needed information from providers and any otherentities working with the individual under guardianship. Monitors conditions set by the court. Works with residential providers, fiduciary staff to ensure payment of bills and provision of personal needs,benefits staff to establish and maintain benefits including Medicaid, food stamps, pensions, federal and othereligible benefits. Other duties as assigned. Skills and Expectations include but are not limited to the following:
Excellent verbal and written communication skills. Excellent computer skills. Ability to plan and establish priorities. Attention to detail, being thorough and accurate in completing tasks. Ability to work cooperatively as part of a team. Maintain a valid driver's license. Investigation, coordinates, recruits, maintains records, and court skills. The successful candidate should be professional, polite, and able to work with a diverse population. Thecandidate should be able to deal with difficult situations in a calm, respectful manner. They should beresponsive, willing to meet each challenge directly, and able to ask for assistance or guidance when needed.The successful candidate should have the drive to meet the needs of those we serve.

EDUCATION: Graduate of a college or university with a bachelor's degree in social work, sociology, psychology, marriage and family therapy or a related field.

Employing agency is responsible for ensuring employee possesses and maintains required licensure(s), certification(s) or other credentials.
Working Conditions
Performs job duties in an office/facility or residential program setting and visits clients and/or families at their residence. Provides emergency services on 24-hour basis, including accepting referrals and completing investigations at any hour without prior warning. As a result of enforced intervention, the worker runs the risk of physical harm.
Probationary Period
This job has an initial and promotional probationary period of 9 months, except as provided in KRS 18A.111.
