Pay:

$90,000 - $130,000 DOE

Schedule:

Monday - Friday plus on-call and weekend requirements.

Serving the Greater Twin Cities Metro Area (21-County Service Area)

What You'll Do

Admissions:

Complete hospice eligibility assessments, consents/education, med reconciliation, immediate symptom plan, and safe-start services.

Case Management (as assigned):

Own a patient panel-plan of care, symptom management, caregiver coaching, IDT coordination, timely documentation.

Visit Nurse/Crisis Support:

Provide PRN, routine, and urgent visits; execute standing orders; escalate and collaborate with the MD/NP swiftly.

Interdisciplinary Collaboration:

Partner with social work, chaplaincy, aides, and therapy to support patient and family goals.

Documentation & Compliance:

Chart accurately in the EMR; follow Medicare CoPs and agency policies.

What You'll Bring

Active Minnesota RN license.

Required hospice background

from one of the following:

prior hospice agency experience

or

high-acuity settings

such as

ICU, palliative care, or oncology .

Strength in admissions interviewing, symptom triage, and family education.

Organized, decisive, and comfortable driving across the service area (reliable vehicle/insurance).

Professional, compassionate communicator who thrives in a dynamic environment.
