We are seeking
two experienced RN Care Managers
for a temporary 13-week assignment. The RN Care Manager plays a critical role in coordinating care and services for patients during hospitalization, ensuring safe and effective transitions of care, and acting as the primary liaison between patients, providers, payors, and community resources. Key Responsibilities:
Perform comprehensive patient assessments to determine care needs and transition planning Lead and collaborate with interdisciplinary teams during daily care rounds and planning meetings Communicate effectively through written, verbal, and electronic systems Coordinate discharge planning, including arranging DME, transportation, and insurance authorizations Manage concurrent utilization review and ensure appropriate resource utilization Monitor and influence patient length of stay Understand and apply knowledge of hospital reimbursement and quality measures Qualifications:
Education:
Associate or Bachelor’s Degree in Nursing (BSN preferred) Experience:
3–5 years of clinical experience in:
Medical/Surgical Nursing Home Care Hospital Case Management Utilization Review (required) Discharge Planning (required)
Licensure:
Active RN license Certifications:
BLS (required) CCM (Certified Case Manager) or ACM (Accredited Case Manager) preferred
Preferred Skills:
Strong interdisciplinary team leadership Familiarity with discharge planning resources and local community supports Knowledge of hospital-based care management, utilization review processes, and payer systems About Talent4Health
