A reputable organization that drives solution to transform the lives of people with disabilities, people in poverty and people suffering from substance abuse/addiction, has an immediate need for a Registered Nurse (RN) for there supportive housing program.

Compensation: Pay Range: $50.00-$55.00 Per HR
Description: Collaborate with mental health professionals on care teams. Manage diagnosis and treatment of health issues through case management. Offer health teaching and counseling, including group sessions. Implement medical regimens prescribed by healthcare providers. Provide interventions that support or restore well-being.
Key Responsibilities: Assist clients transitioning from hospital care to community living. Ensure clients connect with and engage community providers. Conduct health assessments and manage cases. Provide health education and counseling services. Collaborate with healthcare professionals to execute medical regimens.
Qualifications: Valid NYS RN license. At least 1 year of experience with chemical dependency as a primary diagnosis. Excellent written and verbal communication skills. Knowledge of office productivity tools. Ability to provide crisis intervention with strong interpersonal skills.
