Description: The Integrated Case Manager for Population Health is an essential member of the patient-centered care team. This role is responsible for assessment, planning, facilitation, care coordination, evaluation, and advocacy. The goal is to meet individuals' and families' comprehensive health care needs. This is achieved through effective communication and the utilization of available resources. Promoting patient safety, quality of care, and cost-effective outcomes is vital. The Integrated Case Manager addresses the needs of patients requiring complex management strategies. Services are provided to patients in various settings, including ambulatory and inpatient environments.

Key Responsibilities: Conduct comprehensive assessments of patients and families to gauge biomedical, psychological, social, and functional needs. Develop personalized, patient-centered care plans to enhance the care experience. Engage patients and families as part of the care team through advocacy and education. Utilize professional judgment and critical thinking to help patients overcome barriers. Provide counseling related to treatment decisions and end-of-life issues. Ensure seamless transitions between care settings. Advocate for appropriate delivery of services within the health plan. Collaborate with the care team to manage patients with complex needs. Evaluate and modify care/treatment plans as necessary. Participate in process improvement activities designed to reduce risk. Address cases involving abuse or violence. Support department goals that contribute to the organization's success. Perform other assigned duties as needed.

Candidate Profile: Candidates must possess strong interpersonal skills and customer service orientation. Knowledge of preventive service guidelines, clinical practice guidelines, and case management principles is crucial. Understanding social determinants of health and their impact on patient well-being is essential. Strong organizational skills with the ability to manage multiple complex patient needs simultaneously are required. Demonstrated compassion and advocacy skills for patients and families are necessary. Experience in discharge planning, home health care, rehabilitative medicine, community health, or managed care is preferred.
