Job Description

The Integrated Case Manager for Population Health is a key member of the patient-centered care team responsible for the collaborative practice of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet the comprehensive healthcare needs of patients and their families.

Responsibilities

Conduct comprehensive assessments addressing biomedical, psychological, social, and functional needs. Develop personalized patient-centered care plans aimed at optimizing the patient experience. Engage patients and families through advocacy, ongoing communication, and health education. Utilize professional judgment and motivational interviewing to assist patients in overcoming barriers. Provide counseling and interventions related to treatment decisions and end-of-life issues. Coordinate patient transitions between care settings for seamless and safe experiences. Advocate for appropriate delivery of services within the patient's health plan. Collaborate with the care team to manage patients with complex medical and social needs. Evaluate patient responses to care plans and make necessary modifications. Participate in process improvement activities to reduce risk and enhance patient care. Facilitate interventions in cases of abuse and neglect, as necessary. Required Experience / Certifications / Licensure

Minimum 5 years of Acute Inpatient Case Management experience is required. Strong computer skills and Epic EMR experience are needed. A Nursing degree or MSW is required, along with valid Michigan licensure (RN or LMSW). BLS certification through AHA is necessary. Recent experience in acute inpatient case management is required.
