Description Assess patients in inpatient or outpatient environments and craft personalized treatment plans Implement physical therapy interventions targeting mobility, strength, balance, and functional capacity Collaborate with nurses, physicians, case managers, and rehabilitation specialists to achieve optimal patient outcomes Guide patients and their families through therapeutic goals, discharge strategies, and home exercise plans Document evaluations, treatments, and patient progress in line with facility standards and regulatory requirements Participate in interdisciplinary care team meetings to ensure seamless care coordination Education
Doctorate in Physical Therapy (DPT) or an equivalent credential from an accredited program Certifications
State licensure as a
Physical Therapist CPR certification preferred Additional specialized certifications (e.g., neurological, geriatric) are beneficial but not mandatory Skills
Prior experience in a similar setting is highly preferred Ability to thoughtfully care for patients with complex medical and mobility concerns Outstanding communication and teamwork capabilities Proficiency in utilizing digital charting and clinical documentation systems Benefits
