Description Conduct assessments of patients in inpatient or outpatient environments and create personalized treatment plans Deliver physical therapy interventions tailored to enhance mobility, strength, balance, and function Work closely with nurses, physicians, case managers, and other rehabilitation professionals to attain optimal outcomes Guide patients and their families in understanding therapeutic objectives, discharge processes, and home exercise routines Document evaluations, treatment sessions, and progress meticulously in line with facility and regulatory standards Engage in care team discussions and interdisciplinary meetings to ensure seamless patient care Education
Doctorate in Physical Therapy (DPT) or a comparable degree from a recognized program Certifications
State licensure as a
Physical Therapist CPR certification is preferred Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory Skills
Prior experience is strongly preferred Ability to manage patients with complex medical and mobility challenges Excellent communication and collaborative skills Comfortable utilizing digital charting and clinical documentation systems Benefits
