Key Responsibilities Conduct evaluations in both inpatient and outpatient settings and create personalized treatment plans Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and function Collaborate with nurses, physicians, case managers, and various rehabilitation professionals to achieve optimal patient outcomes Guide patients and their families on therapeutic objectives, discharge procedures, and home exercise regimens Document evaluations, treatments, and progress thoroughly in line with facility and regulatory standards Engage in care team discussions and interdisciplinary meetings to ensure seamless patient care Qualifications

Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution Certifications

State licensure as a

Physical Therapist CPR certification is preferred Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory Core Competencies

Preferably possess previous experience in a similar role Capable of treating patients with intricate medical and mobility challenges Strong teamwork and communication skills are essential Familiarity with digital charting and clinical documentation systems is a plus Benefits of Working With Us
