Description
Position Summary: We are seeking a compassionate
Linkage Navigator/Peer Supporter
to help individuals affected by opioid and stimulant use disorders. The role involves reducing barriers to care, facilitating referrals, and supporting clients through case management and harm reduction services. The ideal candidate will have lived experience with substance use disorder and a certification in peer support.
Key Responsibilities:
Facilitate referrals and warm hand-offs to care services. Maintain accurate client data and submit monthly reports. Integrate naloxone and fentanyl test strips into the community. Provide person-centered care to support recovery. Collaborate with local partners and educate the community on substance misuse prevention. Ensure services are culturally accessible to all clients. Requirements
High School Diploma or equivalent. Lived experience with substance use disorder and peer support certification. Strong communication skills and knowledge of local services. Ability to work independently and with diverse populations. Benefits
