Program Specialist 2
- Home & Community Based Services In-Home Support Specialist The Program Specialist 2 performs administrative, direct, and in-home supportive services including duties such as limited in-home services, medication reminders, glucose level checks, blood pressure checks, medical appointment setup, and various HCBS supportive services to enable the elderly and disabled to have a full range of services in their current living arrangement. Metropolitan Social Services (MSS), a department of the Metropolitan Government of Nashville and Davidson County, focuses on topics and issues affecting the economic and social wellbeing of Nashvillians. National and local policy choices have affected living standards and poverty, which can be multidimensional and result from long-term processes of inequality, exclusion, and social and economic factors. Typical Duties Provides appropriate welfare checks as referrals are made and follows up with all designated parties. Provides minimal in-home support services to eligible customers (set up medicines, make appointments, check mail, take out trash, etc.). Provides minimal shopping assistance (grocery and pharmacy) within a 5-mile radius (510 items max) on an as-needed or emergency basis. Creates appropriate case management referrals to the appropriate MSS unit when needed. Provides emotional support to customers and encourages independence. Maintains and stays abreast of certifications and trainings related to elderly, disabled individuals, mental illness, trauma-informed care, and HCBS. Develops and maintains a routine visitation schedule for customers. Completes, monitors, and submits all medically documented logs (glucose, blood pressure). Coordinates with HCBS unit staff to ensure wrap-around services are met and that appropriate plans are implemented and monitored by the team. Works with the health department and other metro departments to ensure health and safety of seniors and vulnerable customers. Ensures customer safety and confidentiality at all times. Documents and submits actions and interactions as required. Attends all required meetings, trainings, in-services, and workshops. Develops and conducts training for unit staff and the agency when needed. Works independently and with the team to address the diverse needs of customers. Establishes, maintains, and nurtures partnerships. Participates in building closure teams to address customers' immediate health needs. Performs other duties as assigned.
Minimum Qualifications
Bachelor's Degree from an accredited college or university. Licenses Required
Driver's License. Preferred Experience, Knowledge, Skills, And Abilities
Experience working with the elderly and disabled population. Certified Nurse Assistant (CNA) Certification.
